What should I bring with me when I come for an appointment?
When you come for your appointment remember to bring the following:
- Insurance information
- Driver’s License
- Reports from previous operations or imaging studies
- Imaging studies such as X-rays, MRI, CT scans, etc. Please bring the disc with your images. You can contact the imaging location for a copy if you don’t already have one.
